Medical Plan Info
Per the Employee Benefits Dept:
"If you want to make a change to your current medical plan, you will need to complete a medical change form (2 pages). You may download the form off of StaffNet." http://staffnet.cps-k12.org/Staffnet/EmpBene/
"The forms can be found under the Medical/Dental Open Enrollment link. You can also look at the Frequently Asked Questions and the Employee Contributions on this link. The Humana Benefit Summary can be found also in the "What's New" box on the right side of the Employee Benefit web page."
It is CFT/ACPSOP's understanding that if you do NOT turn in a Medical Change Form, you will be defaulted to whatever plan you currently hold.
